"Continued Mistakes"

So this was the second time I came into have my teeth extracted. And again it should have been all four wisdom teeth and yet again it only had two on the paper work. How am I supposed to trust anyone who is about to cut into my face if they can't even get the paper work correct!

And this isn't the first time the trust has neglected me whilst under their care. They failed to correctly diagnose me so shipped me off to Addenbrooke's and if not for them (Addenbrookes) I would have died!
