"Discharge from hospital of my mother"

My mother was taken to the hospital on Tuesday after having a fall, she is 88 years old. I was told on Wednesday afternoon that she was behind discharged and they were arranging transport. She came home with a red cross car and we were very grateful to them. The problem was that she was sent home in the hospital gown, no underwear and a short jacket which did not cover her properly. Mum said they did not put her clothes on or use her dressing gown because it was short notice. I also asked if they had arranged for the district nurse to come again to administer her insulin as she has diabetes, macular degeneration and dementia I was told that this was being arranged. The following day the nurse did not come as they had not been informed of the discharge. My mother did not have her insulin until 3. 30pm. I do appreciate that the staff are busy but this is not good enough. I am sorry to have to complain but I hope this does not happen to anyone else.

We are very sorry to learn that your dear mother has had this experience. We are particularly concerned that in addition to the problems with district nurses attending following your mother’s discharge, your mother’s dignity and privacy seems to have been neglected.

We are keen to identify the service area in which this failure occurred and use your mother’s experience as a learning opportunity to understand what happened whilst we were delivering your mother’s care and to take steps to ensure it doesn’t happen again.
