"Integrated Care"

I have been receiving care from members of the Integrated Care team, A Community Matron, Occupational Therapist and Self-Care Advisor. My O. T. Karen has brought in every piece of equipment avialable to me and has totally changed my life. She has been so understanding of my needs I just cannot blieve the difference it has made to my life. I am also agrophobic and she has even managed to get me out of my back door for the first time in four years. I still cannot believe it. What can I say as I have told them myself the word Thank You is not enough the three ladies are wonderful and my life has been greatly enriched from their care.
