Why do you send out forms for patients to complete prior to outpatient appointments and the form is not asked for by any of the three different staff, and they all ask the patient to answer those same questions regarding their health that they have completed on the form?

It is inefficient and expensive.

I realise the staff need to ask name, date of birth and first line of address to check they are dealing with the correct person, but to repeat the health questions three times is ludicrous.

Why not just read the form?

Why do your appointment letters tell patients to bring in their medication, yet not ask to see it, instead staff ask for a list.

When a patient has complex conditions that require 19 different medications this is unnecessary.

If a patient reorders their medication online at the GP surgery they do not have a GP prescription to bring in.

It is very confusing for elderly or disabled or very ill patients, and really stressful too.
