"incorrect medical report submitted"

About: Castle Hill Hospital

I went for an ultra sound scan in April at Castle Hll and by June when i still hasn't received any information, rang my GP who informed me there was no report issued

several phone calls later backwards and forwards from GP to castle hill, i was told the report has been sent to GP (it hadn't). I then made an appointment to view the report at my GP surgery only to find that the consultant had reported that i'd been given steroid injections after my ultrasound scan. This is totally incorrect and mis reported to my GP who then didnt follow anything up, so i'm another 4 months down the line with no medical help. Outrageous
