"Poor Management"

I have been a Glaucoma Patient for several Years now, the Last Appointment was JUNE 2015, a Year ago, I was told a Year ago that I would receive another Appointment in 6Months which is now 6 Months ago, I received a strange Form asking me "Do you still need an Appointment" I found this Extremely Strange when I am under the Specialist for my Eyes and they said last June that they needed to see me in 6 Months, then they ask me for my opinion! I did send the Form back saying yes I do need an Appointment but have still not received one, As my Eyesight is at risk I find these circumstances very disturbing !
