Please can the continence service consider if a patient has dementia before they send the complex sheets to fill in before an appointment. My Dad has dementia and there is no way he can complete the sheets, my Mum is anxious, stressed as his carer and has short term memory loss and is now even more stressed about having received the sheets to complete even though I have told her not to worry about them. It just doesn't feel very dementia friendly.

As a service we deal with many patients who have dementia and our Community Team of health care assistants and staff nurses have undertaken training in dementia awareness and are 'Dementia Friends'.

On referral we are usually able to gather information about other health issues a patient may have which would effect their ability to complete diaries. Contact details of a carer of relative with whom we can liaise should have been taken. It appears that this has not happened in your dad's case.

The diaries are really useful as part of the bladder and bowel health assessment. They give staff a full picture of the continence issues patients face and enable them to put in place a treatment programme and plan of care.

Please accept my apologies and my assurance that when you dad is seen by our community team he will receive an holistic assessment that will be dementia aware.
