"District Nursing at Bull Farm Primary Care Centre"

(as a staff member posting for a carer/relative),

patients husband advised he feels the heart failure nurse and the community nurses did his wife proud. The heart failure nurse discussed aspects of care in a compassionate manner with the patient and her family to ensure she received the end of life care she wanted. The patient was able to return home and experienced a peaceful end to her life surrounded by family, without support from the heart failure nurse, community nurses she would not have been able to remain at home.

I'm sorry to hear about your loss. Thank you for providing this feedback, it's great to hear about the positive impact that the Heart failure Nurse and the Community Nursing team had on your wife's care and that she was able to return home.
