"no care plan in place for when I went home"

In October 2014 I was informed by The sister in Godstone Ward at Surrey & Sussex Healthcare trust ( East Surrey Hospital) that an ambulance was going to collect me and take home on the following morning at 10 am.I was taken home by ambulance to an empty house and my daughter had been informed that I was being sent home at 8. 00 p.m. when she visited me on the ward.

No advance warning of this eviction was given to me or my daughter and no care plan for discharge ever discussed or agreed.

On this evening in October 2014 a social worker from Surrey County Council arrived on my door and said they were going to put me to bed and get me up each morning and feed me three times a day.

As a result of this eviction after two months in the hospital ward and failure to care for me I was registered disabled by Surrey County Council in November 2015. I complained to the PHSO and in my view they did not investigate the complaints against either body competently. I am now reporting both bodies to the Care Quality Commission for on going breaches of the legislation know as the Duty of Candour.
