"Poor district nurse to doctor communication"

Having had 4 days in hospital, being discharged Sunday evening, on my head 13 week ago I had a cancerous growth removed. This has been dressed 3 times per week since and still need dressing. Sunday the hospital changed the dressing, offering to arrange for district nurses to visit. I said that I would arrange this myself. Monday, I contacted my own doctor who supposedly arranged for visit Wednesday, Friday and Monday. No body has yet been. I phoned last evening and was told some body would be here today. Phone call to say will not be here until tomorrow, the person would not listen all she would say is tomorrow. I take immune system represents for a ongoing complaint and am very susceptible to infection, that's why I have been in hospital. Who is responsible if I end up back in hospital?
