This is the third time I have filled in this box, because I had to start again each time even though it was only my 'screen' name was lacking.

I have been trying to get through to the hospital on several occasions, each time holding on for ages and then being either cut off or being told that my call could not be completed so try later!

How do you manage to cancel an appointment if you can't get through to do it.

It just means you don't turn up because you can't, so it's a wasted appointment for you, costing the NHS money when hiring someone to answer the phone would stop this happening.
