"intermediate care services"

About: Coventry And Warwickshire Partnership NHS Trust

(as a carer),

My mother had a fall on the 28th December and the paramedics were called and dressed a cut on my mothers head. She remained at home and she was informed that a GP would call that evening. The GP visited soon after and ensured that mum was ok.

Next day a falls nurse called and mum was checked again. I asked the nurse if there was help available which would assist mum with tasks of daily living. The nurse immediately contacted ICS and a carer was arranged for that evening. The following a nurse visited to complete a care plan. Unfortunately mum was admitted to hospital on the 31st December 2015 with a UTI, but the care prior to her admission was provided in a very short time and was appropriate for her needs.
