"Breakdown in communication about my father's care "

My father suffered a stroke back in February 2009 and was admitted again in December 2009 for an ultra sound scan.

He was suffering pain in his testicals. He had to wait two days to get this done and whilst i was visiting there,I saw what I though was a total lack of organisation and care that was bordering on scandalous.

Several doctors saw him and no one seemed to know what the problem was, even though they had written their notes, god knows how many times.

He was transfered for surgical admissions late at night to Poldark ward, without anyone on that ward even knowing what was wrong with him. We had a call the next morning saying my dad was confused and couldn't tell the doctors what the problem was. Well of course he couldn't as his speech had been severely impaired by his stroke earlier on in the year.

How can there be such a break down in communication?

On the day he was coming home we had a call to say he was very unwell and had another major stroke.

My family rushed to his bedside to find him alone unconscious and covered in vomit. They didnt even have the decency to clean him up before we came to see him die.
