"Poor record keeping"

I have been taking the same eye drops for 15 years , 1 drop in 1 eye once per night. I saw a doctor for my check up and they gave me another new drop for both eyes, i told the doctor i only take them for 1 eye, the doctor said their records show i should be taking drops in both eyes.

When i went back for another check, i told the doctor i didn't like the new drops, they put me back on the old drops, and i said in both eyes , the doctor said no, why ?

My wife was waiting for laser eye treatment , she waited about 9 months , when we were told 3 to 4 weeks...When she did go for treatment after being there most of the day, the doctor came to operate and looked at their notes , and said the appointment she had was for cataract operation when she should have had an appointment for laser treatment. Now we have to go to all the way to Worthing for her treatment.
