"Ward moves"

About: Queen Alexandra Hospital

It would be helpful to inform relatives that the patient has moved ward, 5 times in 10 days is also a concern for continuity of care. Pressure area care for virtually immobile 83 year old discussed and barrier cream offered on several occasions without materialising, I would have thought pressure mattress for # pubic ramus would be beneficial . In casualty many great individuals but introducing by name and role would be useful. MAU consultant great and suggested that patient was best remaining on that ward so not starting again, this has then happened 5 times.

Patient told yesterday that the staff would refer under section 2 to social services, she said does not want a referral, this is because she did not understand what it means, no discussion with family despite power of attorney for health and welfare, and when I expressed my concern about discharge straight home, told that patient has capacity, not sure that this has been assessed , and she does have memory loss and would be happy that decisions are made with family who visit every day and have medical knowledge
