The programme of advice in patient information booklet nothing like that delivered on ward 5. The nurses advice re. bed transfers nothing like physio's advice. No one dressed on day 3. 52 bed Nightingale wards - just like when I started work for the NHS 35 years ago - can't believe they still exist! No assessment of main carer - my dad .Training and advice re how to care only given by O.T when I requested on day of discharge. Poor communication between nurse and O.T - my concerns were not relayed to O.T by the nurse/s. Very rude charge nurse - despite raising concerns - I was told check list completed, O.T signed off mum as fit for discharge (although not informed of concerns in regard to deterioration in my mums memory and mental health) . Nurse stated my mum was medically fit - tachycardia and temperature from day previously had resolved - however within 40 minutes of phone call she had a raised temperature/tachycardia and concern re. infection and not ready to be discharged?? Obviously check list not completed before discharge agreed? Do it yourself OT , Do it yourself Physio and Do it yourself nurse - 74 year old father told to inject mum with anti-co-agulation medication for ? a month - no written guidelines and no demonstration - incredible!! And no follow up to monitor discharge etc!!!!A specialist hospital ...no. Not even practicing evidence base or National Guidance.No ITU or medical support re. deterioration in condition or emergency care as advised re. NICE guidelines re. Hip Replacement. All together very poor
