What could be improved

I was supposed to have been sent a letter for an echocardiogram after referal by my GP. I did not even receive this letter until after the appointment they had booked for me because the hospital had sent the letter to the wrong address! The letter was also over a month late and it said if I missed my first appointment I would have to go back to my GP for another referral! This hospital is an absolute joke and needs to get their admin in order.
