"Be careful staff putting wrong readings down on..."

My husband has a chronic ongoing breathing problem and can go down very quickly so the observation are very import as an indication, time and time again we witnessed staff putting down a slightly lower reading when his observation required a Dr to be called. This got so bad that when I arrived to visit On one occasion I insisted a Dr was called as he clearly was unwell and once a Dr came and the blood gases were taken it confirmed my husband was very unwell and hypoxic and he'd said his sats had been low all morning so when I checked the charts they had be changed to ok readings, we can't complain because we will need the services again at some point but please don't leave your rels here it's not safe stay with them and check that the obs are being recorded correctly it's very dangerous. The staff need training badly.
