"Given wrong medication"

About: Southmead Hospital

I was given a medication prescribed by this department but this has worsened my symptoms despite the consultant ensuring there would be no problems with me taking it. Surely all other underlying conditions should have been investigated first before issuing me with such medication?

Dear J – I was sorry to her that suffered an adverse reaction to your medication. Sadly, it is not always possible to predict when a patient will suffer an adverse reaction, despite advice from the pharmacists, and the pooled knowledge of the clinical team; I regret that this was your experience. Please be assured that patients are monitored at all times for just such reactions and mechanisms exist to ensure the lessons learned are used to inform the British National Formulary, where appropriate, to expand the understanding of possible side effects to all medications.
