What could be improved

Bought in to AE with cardio problem. I was mixed up with another patient so doctors and nurses couldnt find me inthe Ward.

I was told I needed a 24 hour cardio monitor which would be arranged asap. I have tried to follow up to find out when. Perhaps over a year is not considered a long time and perhaps chronic heart disease is not considered serious enough to warrant any record keeping.

Anything else?

I have received correspondence from the trust asking for urgent attendance at radio therapy. it was sent to my home address but the name was entirely wrong. I did contact the hospital but was told not to take any action.
