" Patient details need checking"

My daughter in law was admitted into hospital this month to have a baby, and was in for about a week. During this time no-one asked if her details were correct on their notes. On leaving the hospital the ward obviously informed who they thought was her GP, that she had been discharged.

The worrying thing is that a Community Midwife from the wrong practice actually came out and visited, and never asked why she had not had any antenatal care with the practice!

Thanks for your post, I'm sorry you have been worried about your families care.

Patients’ details are checked on discharge. This includes home address and GP, and this must have been done for the Midwife to go to the right address, so please be reassured that the details will have been checked.

The confusion may have arisen as Community Midwives work in teams covering each other on days off, annual leave and weekends. Therefore a woman may not get a Midwife from the Practice she is registered to but will be passed back to her named Midwife when she is next on duty.

I've passed the information on the Community Midwifery Service, and they are going to look into producing Team leaflets with photos, so that in the future, misunderstandings such as this will not take place, and women will be able to see at a glance the whole team they may come into contact with.
