"My mother's care at Lister Hospital"

(as a relative),

We called an ambulance for my mum due to difficulty in breathing. They did an X ray blood tests and checked her oxygen levels which were very low. Unfortunately, the hospital missed the most important thing, my mum developed a DVT blood clot which hospital failed to pick up.

My mum was discharged on Monday and lasted 24 hours. My mum passed away on the Tuesday in an ambulance.

My mum's next door neighbour overheard the ambulance man state this was a failed/forced discharge. They also stated it was a blood clot. How can the ambulance men spot it but the Dr's in the hospital didn't?

I would like answers.

Thank you - we've got that email and as you should have seen by now, we have replied telling you that we have asked our senior nurse who covers the clinical team who cared for your mother whsilt she was at the Lister to investigate. It may take her a day or two to get in touch, as she will need to pull all the records and talk to the staff involved in looking after your mother, but she will be in touch soon.

Thank you I really appreciate this. I have questions that my sisters and I would like answered. Is there any possibility of arranging a meeting. I know this is not going to bring my mum back but it might help us as we are very much grieving and in shock.

I remember my mum saying to me on Monday that the pain in her leg had gone, in hindsight now, this could've been the clot on the move. I'm shocked that my mum was dehydrated which means the blood is thickening, her age and low oxygen levels in her blood, to me further investigations to rule things out.

They just seemed, I could be wrong, solely focus on copd.

Thank you very much for your time, I really appreciate this I really do.

As you can imagine we are in deep shock and it's difficult to come to terms with this.

We're sure that a meeting can be arranged - best to ask our nursing colleagues when they make contact as they will know who to involve so you and your sister get the answers you need.
