"Lost Proctogram Report"

About: Royal Alexandra Hospital / General Medicine (wards 8, 10, 11, 14, 18 & 27) University Hospital Wishaw / Gynaecology (Ward 13)

(as the patient),

I had this procedure at the end of 2014 after waiting 7 months. I waited 12 weeks after hoping to hear from my consultant at Wishaw General. They said they hadn't received the document that was 'mailed' (which I took to be emailed otherwise the person would have said 'posted') at the end of 2014.

I spoke with member of staff recently and they agreed to investigate the disappearance of this vital document.

It may be too soon for the member of staff to investigate I concluded however, this failure needs to be highlighted since my review of a prolapsed bowel & bladder cannot, apparently, be progressed without it.

Its over a year now since this procedure was requested and as a result of the delay I now cannot been seen until summer. Since these conditions are huge complication to the Scleroderma & Raynauds that I also have, I am appalled that there are no safeguards to prevent such failures.

I'd be very interested to hear your view on this entirely preventable situation, how it occurred and how you plan to ensure that it never happens to anyone else.

Dear Madge51,

I’m sorry to hear of the difficulties you’ve been experiencing. I’m aware that you are in contact with my colleague Susan Friel (Chief of Nursing Services) at Hairmyres Hospital about your investigations and treatment.

I’ve asked one of my Senior Nurses to look into why the report wasn’t received and to liaise with Susan directly. I hope she’ll be in a position to give you an update the next time she speaks with you.

As a result of your feedback we’ll look at the administrative systems and processes we have in place around reporting to see if we can improve them.

Good morning Marion, Whilst I greatly appreciate your reply, as stated, one of the staff in the radiology department was already on to this.

It is almost a week since I spoke to the department and I would be grateful if the issue I raised could be addressed directly with me. By all means copy Susan Friel in but since I raised the situation I would appreciate being communicated with directly and not through any third party.

I look forward to all of my questions being answered in detail in order to unravel the ensuing miscommunication that seems to have taken place since others became involved. I refer to a member of staff who seemed to be under the mistaken impression that Andrew Pover was no longer analysing proctogram results, whereas your radiology department told me he was.

I would also appreciate confirmation of when a copy of the report was faxed to my GP surgery because as of Tuesday 4.30pm they had no record of it.
