"My A and E experience"

About: University Hospital Wishaw / General Surgery (Wards 16-18)

(as the patient),

I called NHS 24 recently at 11pm. My arms had swollen up, I had intense pain across my chest bone, scapulae and my arms had little lumpy bits in them. I had a sore throat, blocked nose and was having trouble swallowing.

The nurse I spoke to, after the call handler logged my details, told me to take some paracetamol and that I would be seen this evening. I was called back and an appointment had been made with the out of hours GP service at Wishaw.

My father took me to Wishaw for 12. 30am. I was seen by the GP. However, when I told GP I also had Addison's disease, the GP attributed my symptoms to Addisons and then said they couldn't do anything and that I should see my endocrine specialist.

I told the GP I wasn't happy as I don't really understand why NHS 24 had referred me to someone who doesn't want to deal with any of my health needs because I have Addisons. The GP dismissed me and said they would speak to a physician.

The GP then appeared, presented me with a letter and told me to hand in at reception. Moments later I was taken down a corridor and I found myself in a ward. When I asked what was happening, I was told I was being admitted. I asked for what and I was told that my GP had requested tests and indeed, the request looked like it had come from my GP surgery. The principle diagnosis code used was "shoulder pain". However, it was not from my GP but the out of hours GP.

I was admitted, had an ECG and blood were taken. What bloods? I have no idea. I asked that someone let my dad know where I was and bring me handbag and it had my emergency medication. They did and they sent my dad home but took his number to call him to come get me when I was discharged.

I fell asleep. A doctor came and asked me some questions. I was fast asleep. The doctor poked me in my left shoulder and asked me if it hurt. It did and then I fell asleep. The next thing I know a nurse appeared with a box of cocodamol and a letter. The nurse told me I was being discharged and to get myself home. It was 4 o'clock in the morning.

I have a compromised immune system and at the very least have an upper respiratory tract infection. I live with and care for my mother who is having chemotherapy and has a compromised immune system. I now do not have access to my GP - no appointments free today. I can't see GP at out of hours as they 'don't do Addisons' and I don't want to be treated as someone who is being 'punished' by being discharged.

So what do I do now Wishaw? Wait until I have an adrenal crisis or my mother gets this too and ends up ill?

Dear Kikicee

I am very sorry to hear of you recent problems with your arm and respiratory infection. From what you have said it seems to me you have had a difficult journey with poor communication on our part.

To allow me to investigate this matter I would be grateful if you could contact Yvonne Ross our Patient Affairs Manager by calling 01698366558 with your details I would strongly encourage you to do this please as this is not the standard of care I would expect at Wishaw General Hospital. I am unclear where you were admitted to from your posting therefore more information would be helpful.

As to what you do now please contact your GP urgently and he can refer you to the correct specialist for your condition. If your condition does deteriorate today then I would suggest you visit the ED department.

I am sorry to hear about your mum this must be difficult for you when you are not feeling well either I wish her well. If you contact us we will be able to advise you how to get assistance if you need help caring for your mum.

Thanks for your prompt reply. However, this happened on Friday, 13th March and I've done what I can to get the care I need. It felt pointless going back to Wishaw or the GP. I made an appointment with a GP privately and was prescribed a course of doxycycline and was referred to an ENT. The pain I was experiencing was because sometimes too much steroid (e.g. When one has to due to infection etc) can cause painful inflammation of the cartilage, especially around the clavicle. He also confirmed that I was safe to be around my mother; this and a potential adrenal crisis were my primary concerns.

I shall make contact with Yvonne Ross tomorrow, as I would be keen to help ensure that this doesn't happen to someone else in my predicament. I would also been keen that staff understand Addison's disease, to support me and others in the best way.

Thank you again for replying. I don't know the ward number but I'll try to pull and many details together before I do speak to Yvonne.

Dear Kikicee

Thank you for responding I am sorry for the distress this matter has caused you and your mum I wish you would have raised this matter at the time with me or my team to allow us to get you the help you needed. Should you ever have any problems in the future at Wishaw General please raise them at the time with the staff and if you are not getting the help you need contact Yvonne.

I am pleased you are going to get in touch as you say we would not want this to happen again to anyone I would appreciate all the information you can provide.
