(as the patient),

Past 4 years, change of surgeries because of wrong patients details profile been sent out to me etc

Changed surgery, yet again I had another persons name & identity given to me at that surgery. as likewise leaving there as going to a NHS orthotics centre. details I was told that had been written was that I had banged or dropped something on my toe on purpose etc before leaving yet again I had been given someone else personna.

Changed of surgery again. I explain to the new Dr the issues I have with my feet & other issues.
