"Problems discharging my mother"

About: Nottingham University Hospitals NHS Trust - City Campus / Older people's healthcare

(as a relative),

On admission to ward D3 at Nottingham City Hospital my mothers dressing gown was lost. We informed D3 who then transferred us to C54. We informed them also but still it wasn’t found. On discharge the social worker informed me that the ambulance would ring to say what time my mother was expected to return home. But there was no phone call as there was a problem entering the property. The service that night didn’t seem to be in place. I stayed to make sure, when it was apparent that no one was coming. I then called Rainbow Care who informed me that no one had informed them. If my mother was on her own it would have been a failed discharge as she was in hospital for two weeks and a bit disoriented. She had been diagnosed with dementia too. However, I would like to say that the staff on C54 were excellent so please inform them.
