My elderly granddad (88) was admitted to Abbottsbury ward to be treated for 2 infection. The care he received was good. That wasn't the issue. The issue was his discharge. Abbotsbury ward was being closed, so basically everyone had to go home. Ready or not. Even though granddad could not walk to the toilet unaided or actually do much for him self. He was discharged into my equally elderly grandmothers care. With no thought of how she would get him stood on her own. How she would get him to bed on her own. No re enable ment was put in place. My grandmother was ensured the district nurse and gp would visit the following day. Neither were even aware of his discharge. I feel so annoyed anyone could be treated so poorly.
