"Lack of collaboration between departments..."

Very dissatisfied with the collaboration between departments where a patient has multiple issues.Also a failure to really try and get to a complex and difficult diagnosis.Failure to inform patient of their prognosis and put any plan in place for their death. Complaints department very poor in dealing with issues promptly. Lack of empathy for patient welfare in some circumstances. Failure of consultants to carry out promises endangering patients. Could be improved with vision, love, integrity, honesty and a real sense of duty of care to all patients and more time for collaborative meetings about patients. Fear of litigation is damaging patient and consultant relationship. There needs to be far greater use of other University hospitals when medical diagnosis is not understood. Something has gone badly wrong in the NHS. Shortage of staff,lack of adequate training of junior doctors. Once one went to hospital with a sense of safety now it is a sense of fear.
