"Poor maternity care"

The communication between staff and shifts is very poor with the next shift not aware of what is going on. Staff do not appear to have sufficient training and too many mistakes are being made. My daughter in law was missed off the staff notes at change over and as a result the doctor did not examine her - he did not know who she was. Is this acceptable?
