(as the patient),

I would like to say thank you to all the staff who work at 111 centre for their professional and helpful actions and advice while I have been feeling unwell. Whatever the hour, day or night, they helped, while I'm not having Mental Health Services support.

You have helped in many ways to help me through some horrible times while on the brink of collapse and having suicidal thoughts for which I'm very grateful. I want to express how appreciative I am for your excellent support.
