About: Royal Gwent Hospital

(as the patient),

I was a patient in ward 2D Royal Gwent Hospital.

Admitted in late June for 5 days.

I was not given discharge notes.

I expected these to be sent to Vauxhall Practice in Chepstow, which is my local surgery.

As of today's date Vauxhall Practice have not received my discharge notes.

Please would you fax them to Vauxhall Practice. Many thanks for my treatment and for sending my notes to Vauxhall Practice.
