"A community nurse been paying her a regular visit"

My wife was admitted to hospital 12 mnths ago. and changes made to her medication. 6mnths later she is unwell and again admitted to hospital, discharged and then readmitted.

I am not qualified to comment on her treatment but during discussions with the doctors I discovered that a community nurse should have been activated pay her a regular visit.

This did not happen until after her second hospitalisation when I made sure that it was activated. What a difference! Sandra is competent and efficient and used to dealing with older people. Had she been coming after the the first hospitalisation a year ago the second problem would have been picked up at an earlier stage and the stay in hospital been averted.

Thank you for your posting on patient opinion.

Our community teams work very hard to visit patients like your wife who would benefit from our support in the community to prevent an admission to hospital.

To do this we totally rely on the doctors and hospital wards contacting our community teams requesting our support and providing the relevant patient information.

In Nottingham North East we are working hard to establish regular meetings at the GP practices to discuss any patients who they feel would benefit from our support.

We also have a dedicated member of staff who contacts patients over 65 years of age within 48hrs of hospital discharge to check they are ok and again offer our support.

I was really pleased to see how Sandra has made a difference to your wife's care. Hopefully with these GP meetings now in place our community teams will be more informed of patients who would benefit from our services in the future.
