In my view this death was avoidable. He had an achilles tendon injury. Full groin to toes plaster of paris back slab cast fitted. It was heavy and extremely immobilising. There was no risk assessment, no scan, no anti clot meds, no information about dvt, no instuction as to warning signs to look out for. Nothing! Forcible repositioning of his foot after over 5 weeks of being in the equinus position, he nearly passed out with the pain. An adjustable boot should have been used, to do it gradually. Doctor told me no guidelines in place for outpatients . Not true. I have found DVT prevention policy which in my view should have applied to my husband. His death certificate was issued on his 60th birthday. I feel that this hospital has destroyed our lives.
