Dad was took to hospital following a bad fall he was moved to Harborne ward in june as he lives alone they told me he would n there till I come bacl off holiday 18th June to 2nd July! Phoned ward this morning to c how he was hoping to visit & was told he's not there! Asked where he was reply don't know! Phoned main switchboard told th them said they find out what happened & call back! Found out he moved to holm park care home but no one bothered to tell me! If I went up without phoning I would have a wasted journey! Disgraceful & they call it a super hospital!

We are very concerned to hear that you were not informed of your fathers discharge from hospital on your return from holiday. This is not the sort of experience we aim to provide and can understand the frustration this must have caused. We understand the Social Worker involved has contacted you by telephone this afternoon to discuss your concerns and has apologised on behalf of the social work team and the ward involved for the breakdown in communication. If you have any further concerns please contact via our Patient Advice and Liaison Service (PALS) team who will be pleased to assist you. PALS can be contacted by phone 0121 371 3280, by email PALS@uhb.nhs.uk, via the hospital website http://www.uhb.nhs.uk/pals-form.htm or in person by dropping in between 9am – 4.30pm (Mon-Fri) to the PALS office located to the left of the Information Desk in the main entrance of the new Queen Elizabeth Hospital Birmingham.
