We welcome all feedback and would like to assure you that all comments are taken seriously and acted upon as part of our ongoing commitment to improving patient experience. We are very concerned to hear that you were not informed of your fathers discharge from hospital on your return from holiday. This is not the sort of experience we aim to provide and can understand the frustration this must have caused. We understand the Social Worker involved has contacted you by telephone this afternoon to discuss your concerns and has apologised on behalf of the social work team and the ward involved for the breakdown in communication. If you have any further concerns please contact via our Patient Advice and Liaison Service (PALS) team who will be pleased to assist you.
