"Wonderful treatment at Leeds General Infirmary"

About: Leeds General Infirmary

(as the patient),

I have been attending hospitals for about 11 years now for heart trouble. I am extremely grateful for all the care I have received. The treatment and surgery has been wonderful.

The problem I have is transport. A taxi from Otley and back to the Leeds General Infirmary costs £40, which is a lot out of my pension. Apart from this, I have nothing but praise for all concerned.

Thank you for taking the time to share your experience at Leeds General Infirmary.

With regards to your travel costs. You may be entitled to claim back some these costs. The guidance for claiming back NHS travel costs is as outlined in the leaflet "A quick guide to help with health costs including charges and optical voucher values" which states:

You can get help with necessary cost of travel to receive NHS treatment under the care of consultant, or through a referral by a doctor or dentist, if you:

1. are getting, or your partner gets; income support, income based Jobseeker's Allowance or Pension Credit Guarantee Credit.

2. are entitled to or named on a valid NHS Tax Credit Exemption Certificate.

3. are named on a valid HC2 certificate. (includes travel by your dependent children); or

4. are a war pensioner and the treatment is for your accepted disablement.

To claim a refund you need to fill in a HC5 form and you can get hold of this form by ringing 0845 610 1112. Alternatively, you can get form HC5 from a Job Centre Plus or NHS hospital.
