"forgotten twice follow up treatment!"

I am writing on behalf of my dad who was fitted with catheter and was told that I would receive a letter with a follow up appointment which I never received my daughter rang twice only to be told that they were still waiting for his notes to arrived then finally got an appointment for a trial without catheter which did not work and was told that he would receive a letter with an appointment to discuss other options in approx. 2 weeks it has now been 4 weeks and have made another phone call only to be told that the notes have to be found before they can discuss it - Not happy at all. we wait in anticipation!
