(as the patient),

At 12.15 today I went to have my third set of hearing aid moulds fitted since the 19 December 2013, because they kept whistling.

The Lady Audiologist fitted and tested the hearing aids and was very helpful and explained thoroughly what she was doing. Unfortunately I did not see or ask her name.

Very Good.
