"Father with dementia"

My father waited had an appointment for the 14th January at at 2 45pm we arrived at 2 20 at 15 20 my mother asked why she and my father were still waiting and was told that they had been looking for his file for over an hour and was unable to locate it at about 15 45 they had us in but it was not the consultant but a junior which started to look at my fathers eye without the notes he was then was called away for something for the consultant arrived back to say it had not got worse this was without my fathers file. Not sure how he managed this diagnosis without any paperwork to compare with?
