(as a relative),

My partner ( a proficient cyclist aged 64) was knocked off his bike when a car pulled across the road to go onto B&Q in Torquay. He wasn’t thankfully badly hurt but he did have a bad head bang and nasty headache (his helmet was completely smashed inside and so probably saved him from further injury ) followed by quite extensive body bruising. This happened on Saturday 4th January about 4pm. An ambulance was called ( a smaller van) but my partner who was still in shock insisted he was ok and so they left shortly after questioning him. My partner just wanted to get home so didn’t think to ask the driver for his details.

Could you therefore tell us whether this incident was recorded and if so whether we may have the details of the driver? We would appreciate any help you can give us.

I hope your husband is recovering well after his accident.

It is unlikely that the ambulance crew who attended to care for your husband would have recorded the details of the other vehicle on their paperwork. However, if your husband would like a copy of the record that the ambulance crew completed to document his care, he can request this by contacting our Information Governance team on information.governance@swast.nhs.uk.
