This ward is grossly understaffed, particularly on nights. There is little/no continuity of care and despite problems being highlighted including those around issues of hygiene and infection control nothing is done. Protocol for dealing with contaminated items is not followed putting patients, staff and relatives at risk. Patient records are either not completed or completed incorrectly again putting patients at risk. Until such times as the safety of patients can be guaranteed and the ward staffed with an adequate number of suitably qualified permanently employed staff instead of Agency/Bank staff (qualified and unqualified) vulnerable patients will remain in danger and serious clinical incidents will occur.
