"getting an ultrsound appointment"

I am writing to you regarding my referral for an ultrasound sent by my doctor from Church view surgery Plymstock after I visited him in mid-October.

I received a phone call a week later asking me where I would like to go for my ultrasound.

I opted for Barton Surgery, Plymstock and was told they would contact me with an appointment. I waited several weeks and heard nothing so I rang Barton Surgery and was told they do not arrange the appointments.

I rang Derriford in early November to see if they could help but was told I was not on their list.

I waited until yesterday and contacted my surgery and they checked and said the referral had been sent to Tamar referral service so I contacted you and was given the appointments phone number which I rang.

I was told there was a problem with my referral from the surgery and I would be going to Derriford and he would ring them to make sure they had my referral and he would ring me back before 5 o'clock to confirm I was on their list.

I did not receive a phone call so I rang Derriford this morning and was told they had no referral for me.

Once again I rang the Newton Abbot number. I was told that he had contacted Derriford so I informed him I had rung them this morning and they knew nothing about my referral and I was definitely not on their list and he admitted it had been missed.

I have given this information to my surgery and hope they can do something about it.

Dear ‘Kippy’,

Thank you for taking to time to share your feedback with us, I can only apologise for the significant delay you have experienced. From what you have told us I can tell that your referral has not been processed as we would normally expect and has taken a lot longer than it should have.

I would really like to investigate what has happened and make sure your referral has now been processed and you have an appointment. If you are happy to please contact me on my direct number 01626 883 712, I work from Monday to Friday between 9am and 5pm.
