"Our relative was moved to another hospital without our knowledge"

(as a relative),

We are upset that our relative was moved to another hospital without our knowledge. How shocking for a vulnerable elderly patient who is frightened and very dependent on support from family. The nursing staff were very respectful and caring towards him during his admission but the multidisciplinary team did not include the family in any of the plan of care.
