(as the patient),

I have had my appointments at the above repeatedly changed over the last 2/3 years and this week, I turn up for an appointment (which had already been changed once) to be told it has been changed now to 2 weeks time.

I have been addressed with a different, my telephone number was wrong in your information at one time, when I turned up for an appointment and was told it was the Childrens squint clinic.

should you not move into this century and use email contact?
