"Community transport provision not meeting individual's needs"

My stepdaughter is 19 and has just started using adult services. She has severe learning difficulties, limited mobility, autistic tendencies and can have epileptic fits although this is generally well controlled by medication. When she was in children's services, she had the same transport company and same carer for nearly 15 years. Bearing in mind her autistic tendencies, routine and familiarity is really important for her. Now she's in adult services, suddenly these issues no longer seem relevant or important.

We never know what time she is due to be picked up every week day. The window appears to be 45 minutes which also covers our son's school run. Sometimes she is picked up by a bus, sometimes a taxi and a mixture of the two bring her home. There is never a carer and the taxis who come know nothing about her needs or limited ability to communicate. Her needs are no different just because she's now 19. It seems there is a definite lack of training/provision for clients like her. God knows what they would do if she had a fit when she was travelling. For us and her, this is all very worrying and unsettling.
