My mother is currently under the care of this department and recently had another unplanned admission due to hypercalcaemia. She received treatment on a medical unit to stabilise this but was discharged without the prior knowledge of her consultant and without any instructions or arrangements for future blood analysis. My father contacted the consultant, which is how he found out of her discharge, to ask about further bloods. This resulted in a blood test on a Friday morning the result of which I had to chase up and contact a very frustrated GP who on a Friday afternoon contacted a very frustrated registrar for advice. No consultant available. A decision to increase medication was made although I and the GP felt this was more avoidance rather than any attempt to affect a solution. I feel communication and a planned care pathway was severely lacking in this instance, which could have had very serious consequences for my mother, exacerbating her already compromised health.
