"Poor Processes"

(as a relative),

My 91 y/o Mother requires a urgent blood tests. Twice the District Nurse has attended to take the samples and twice these have either been (a) mis-labeled or, (b) been lost. The District Nurse will attend my Mother for a third time today, causing more unnecessary alarm, anxiety and distress.

I feel that the processes, which should be subject to strict quality control, are in meltdown.

Thank you for bringing this to our attention. You will appreciate that without having more details about your mother and her experience limits our ability to correct any oversight; put in place any improvements we need and to share this with the hospital laboratory too. However, what we have done is to forward your comments to the clinical managers of our community nursing teams to ensure that all our staff are aware of your concerns. We have over 150 community nurses based in teams covering communities across Medway and we are keen to make sure that we continue to provide the best care for patients both in their own homes and at our community clinics.
