"Great care at home"

About: Older People’s Services / Hospital at Home Northern Sector

(as a staff member posting for a patient/service user),

Story by 80+ year old male patient from Derry/Londonderry area

Great care at home- much better than being in hospital.  All were very polite and helpful.  The nurses took great care to explain changes to medications and their importance for my health.

Thank you for your kind response.

Acute care at home aim to prevent admission to hospital at all time. The hospital environment is such a busy place with many unwell patients under the care of nurses and doctors. The acute care at home team attend the patient on a one to one level and are able to give the patient their full attention for that set period. As we work as a multidisciplinary team we work together and assist each other in delivering the best possible care to all.

I will pass on your positive feedback to all the staff.

Many thanks
