Story by daughter of 80+ year old female patient from Limavady area 

My mother found the acute care at home team refreshing, very considerate and professional.  I as her family member had questions answered and was extremely pleased her medication was reviewed as for 18 months her GP/ Nursing Home was just repeating medication.  My mother's medication had never been reviewed and the Pharmacist took her off unnecessary tablets- very happy! Thanks for all your help.
