"A valuable asset to the community"

About: Older People’s Services / Hospital at Home Northern Sector

My 93yr old mother was ill with Congestive Heart Failure and her GP recommended the Acute Care Team to evaluate and treat her at home.

They responded within a short time and after assessing my mom they treated her promptly with Furosemide IV and continued to monitor and treat her for a full week which greatly improved her condition and made her comfortable. The excellent care, attention, professionalism and kindness towards my mom was outstanding. I believe had my mom been admitted to the hospital under todays stressed conditions she would not have received the Acute and prompt care she needed and would not have fared as well. Keeping her at home in her own familiar surroundings treated by these wonderful professionals was the best medicine for her. Special thanks to Dr Todd and his team: Geraldine, Kitty, Barry, Concepta, Robyn and their Pharmacist who kindly called me to discuss my mothers medications.

This team is such a valuable asset to the community and it’s a shame that there are not more of them..they are keeping people out of hospitals which are in critical conditions themselves presently.

I agree the acute care at home team is a very valuable asset to the community, the patients and also their families. We aim where possible to prevent the admission to hospital and I am delighted this was able to happen in your mums case. Nursing a patient in their own home surroundings has shown that the patient improves quicker and are at less a risk of further infections. Families also are able to continue provide any care required.

During our time with your mother all the team were made feel very welcome not only by your mum but by the surrounding family. It was great to see your mum improve and it was very evident how well she was looked after by yourself and your brothers.

Positive feedback for all the team is great and I will certainly pass on your kind feedback to all the team. I hope you had a safe jouney home and your mum continues to improve.
