"Professionalism and good judgement"

About: Urgent Community Response Mid Hants

(as a staff member posting for a patient/service user),

I was referred to the UCR team by my doctor as I was being treated for a chest infection. I was examined and had my blood taken and an ECG recommendation and it was quickly noticed that i would need to go to hospital urgently.  I got sent to hospital on the Friday

In the hospital they found that I had a lot of fluid in my lungs. My GP didn't realise the fluid was building up, but the UCR team did.

I came home with two new conditions and no follow up organised. I felt I had no support. Having UCR completed a safety net.

I was discharged from hospital and my daughter called the GP for the UCR team to come back in and help to make sure I didn't go quickly back into hospital. The GP my daughter spoke to didn't know about the UCR service and she had to tell them so I could get referred again.

Over the 5 days the team got more concerned and identified that I needed to have different treatment to stop the fluid building up again. The team were acutely aware of the issue and Allision and Clare were very experienced nurses and they could see what was happening.

I experienced professionalism from the team and nothing was too much trouble. They took the time to explain everything to my family as well. They demonstrated good judgement and recommendations and intelligence.

If there is only one fault of the service it is that your existence wasn't known. My GP wasn't aware of the service.

I am the consultant for the Mid Hampshire Urgent Community Response service. Looking after individuals at home instead of hospital when it is safe to do so.

Thank you so much for taking time to talk and discuss the care you received from Urgent Community Response Mid Hampshire.

I am so pleased the team were so vigilant and able to escalate your care needs. This seemed to be both before and after you went to hospital.

I will certainly pass your thanks to the team. It is always good to learn from you. I think from what you say we need to do a little more work with our GP colleagues in understanding our service as it's clear not all know about us. Thank you for that feedback.

I will contact the CCG with your feedback and look to distribute our criteria and contact numbers again. This should help with spreading the word again.
